The first step to your STAR Membership is to complete your application and create a STAR PORTAL account. When you have submitted your request you will receive a welcome email with links for required waivers.

You are welcome to participate in STAR programs while your requst is being processed.

Your outside funding source will be charged directly.

The STAR Staff will help facilitate payment through your funding source. Please reach out to your case manager and ask for authorizations to be submitted to the STAR Center.

Memberships can be cancelled with written notification.
